You will be asked to provide your full name, date of birth, address and contact telephone numbers. Pilots will also be required to submit their CAA number.
After Booking
Once you have confirmed your booking, please download the relevant consent forms and useful information to help your medical go smoothly from the section below. Please note all PILOT medicals require that you also complete the Mental Health Questionnaire and bring it with you to your medical.
